Client Site WHS Assessment

This checklist is to be completed when undertaking a client site WHS assessment.  WHS assessments are an opportunity to identify hazards and assess risk to workers and should be completed for all new clients.

Answer each checklist question with either a Yes or No. Further explanation must be provided in the comments section for all No answers.  Once the checklist is complete, calculate the number of Yes answers to determine the Safety Risk Rating at the bottom of the checklist.

Once completed the checklist must be signed by the person conducting the assessment on behalf of the client site.

Risk Rating

Calculate the number of Yes answers to determine the   Safety Risk Rating  below:

Safety Risk Rating: HIGH

Staff may be facing a high level of risk. This level of risk may reflect lack of access to relevant information, but it may also indicate a that safety management should be improved. Changes should be made to the clients WHS safety management system and monitor the situation impacting staff for workplace health and safety. A further assessment should be conducted within 3 months.

Safety Risk Rating: MEDIUM

Staff may be facing a medium level of risk. This level of risk may reflect lack of access to relevant information, but it may also indicate a that safety management could be improved. Changes should be where necessary and monitor the situation impacting staff for workplace health and safety. A further assessment should be conducted 6 months.

Safety Risk Rating: LOW

Contractors may be facing a low level of risk. A further assessment should be conducted within 12 months.
